- Minor injuries
- Minor skin infections
- Minor lacerations
- Rashes
Problems we cannot treat urgently: chest pain, shortness of breath, severe asthma attacks, allergic reactions, life threatening injuries or illnesses, abdominal pain, vomiting/diarrhea and others.
Complex Care Visit: $150
Health Maintenance Exam: $300
Sports Physicals: $50
Please bring required forms.
Medical Weight Loss
Initial visit $300; Follow-up visits $150.
Travel Consultations: $100
In conjunction with Portage Pharmacy.
Hormone Therapy Consultations: $300

- We accept payment via credit card, cash or check.
- We do not accept insurance for these services.
- Payment is expected prior to being seen.
