Walk-in Visits: $75

- Cough
- Ear Infection/Sinus Infection
- Illness
- Minor injuries
- Depression or anxiety
- Minor skin infections
- Minor lacerations
- Rashes
Problems we cannot treat urgently: chest pain, shortness of breath, severe asthma attacks, allergic reactions, life threatening injuries or illnesses, abdominal pain, vomiting/diarrhea and others.
Sports Physicals: $30
Please bring required forms.
Medical Weight Loss
Initial visit $200; Follow-up visits $100.
Travel Consultations: $50
In conjunction with Portage Pharmacy.
Hormone Therapy Consultations: $150
In conjunction with Portage Pharmacy.

- We accept payment via credit card, cash or check.
- We do not accept insurance for these services.
- Payment is expected prior to being seen.
